vision insurance

We are in-network providers with several vision insurances including VBA and VSP. Many vision insurances also have out-of-network benefits that directly reimburse the patient. Please let us know you vision plan infomation when scheduling and we will help you determine how your benefits can be accessed at our practice.

Medical insurance

Many eye exams and office visits are medical in nature. We are in-network with most major medical insurances including Aetna, Capital Blue Cross, Highmark Blue Shield, Medicare, ect. A variety of conditions including diabetes, ocular infection/injury, glaucoma, and cataracts are eligible for medical insurance coverage.

It is your responsibility at time of service to pay any deductible, coinsurance, overage or other fee that is not paid by your insurance company.  ***WE CANNOT GUARANTEE PAYMENT OF YOUR INSURANCE CLAIM. ANY DISAGREEMENT OF ELIGIBILITY OR COVERAGE IS BETWEEN YOU AND YOUR INSURANCE COMPANY.

If you order eyeglasses or contact lenses, you may either pay in full or make a deposit of half to start order. Remaining balance is due at delivery. If using insurance on eyewear overages must be paid at time of purchase.

If you wish to pay out of pocket and receive reimbursement directly from your insurance company, the itemized receipt we give you at the end of each visit contains all the information needed for the insurance company to process your claim. We will happily complete any special insurance claim forms when required.

Our office accepts cash, personal checks, Visa, Mastercard, Discover, American Express, and Care Credit.
